Campus Life Forms

All summer high school students must complete and submit program participation documents. Forms include sections for students, parents and physicians to complete. Click on the link below to download the forms and instructions.

Students participating in the Leadership Institute must submit the Ropes Safety Course Form available in late February.

We recommend that you complete and upload these forms as soon as possible. These forms must be completed and submitted by May 19, 2017 for students admitted by April 24, 2017. Students admitted to the program after April 24, 2017 must submit their forms within two weeks of their program start date.

Students who have not completed and returned their forms will not be permitted to stay in Georgetown housing or attend any aspect of the program. They will not be refunded for any paid fees.

3. Scan and Upload**: Save your 13 page document in numerical order as well as any attachments in one PDF file. Our secure file upload server powered by Box accepts only PDF files.

4. Scan and Email**: Scan then save the form as a PDF, and email the forms with all necessary documents to highschool@georgetown.edu.

**All uploaded and emailed submissions must follow the following file name format: [Last Name]_[First Name]_CampusLifeForms.
Example: Doe_Jane_CampusLifeForms
Submissions with alternative file names will not be reviewed. Photo, Doc, and JPG files will not be accepted.

Apply for Your GOCard (Student ID)

To apply for your GOCard, you must submit a passport-sized photo to the GOCard office by emailing your photo to GOCardphoto@georgetown.edu at least 30 days before your program’s start date. Your email must include your first and last name, NetID, and program name(s). The subject line of your email must include your first and last name and program name(s). For more information on how to submit your photo, please visit the GOCard Office’s Summer Website. If you have attended a Summer Program at Georgetown in the past, please bring your GOCard from your previous program.

In order to receive your GOCard at check-in you must present an official, government-issued photo ID. There are no exceptions to this policy. If you need a government-issued photo ID, please visit DMV.org for guidance on obtaining a state ID. Please note that ID policies vary by state and country. It is your responsibility to arrive at check-in with your government-issued photo ID so please plan accordingly.
